2003 Flat Fee Schedule - (Rev.10/2003)
1. PRIVATE WELL AND SEPTIC SYSTEMS.
Research at MOA/DEC for previous records. Testing of septic system for adequacy, four hour well recovery test (flow & yield), water sample for coliform and nitrate levels, inspection of well for health approval (separation distances), application to MOA/DEC (does not include permit fee or pumping). 2. PRIVATE SEPTIC ONLY (COMMUNITY WATER SYSTEM).
Testing of existing septic system for adequacy and MOA/DEC approval. Includes record search, testing, application to MOA/DEC (does not include permit fee or pumping). Eagle River.....$ 695 Anchorage....$695 Wasilla.....$ 595
3. PRIVATE WELL ONLY.
Water sample for coliform, arsenic and nitrate, well recovery and flow test meets FHA (four hour equivalent), inspection and application for MOA/DEC approval (does not include permit fee).

Well flow test only (for buyer information or second opinion), deduct $ 50.
4. WATER SAMPLES ONLY.

For Anchorage and Wasilla, dedicated trip charges may apply. Note: coliform samples expire in 90 days, Nitrate in 6 months.
5. ASBUILT SURVEY (lots not exceeding 1.5 acres).
In combination with any of above.....$ 375 Survey only..... $ 395 (an asbuilt survey is now required for Health Approval)
6. APPLICATION FOR ANY MOA/DEC WAIVERS REQUIRED
As per hourly fee schedule, we will quote on a case specific basis.
MOA Health Authority Approval (Blue Form) fee is an additional $375.
Septic tank pumping is required by MOA for Municipal HAA after completion of the test on any septic tank not pumped within the previous year's time period (Eagle River/Anchorage $115, Wasilla $125-150).
Septic systems that have not been installed to MOA/DEC approval, have separation distance problems, or were installed without monitor tubes will require additional testing or installation of monitor tubes.
Flat fee testing assumes a five hour well test period and no unusual conditions encountered which require extended testing, beyond a full day's time period to determine adequacy of the system. The above fee's do not include follow-up inspections to correct deficiencies or any meetings or phone meetings with 3rd parties involved in transaction. Any work performed in excess will be on a time and materials basis and only with our client's prior knowledge and approval.
An asbuilt survey copy is required for all Municipal Health
Approvals. Existing survey copies may be adequate for this use.
